*All requests must contain the following information. Missing information could delay your request.
- Your full name
- Any former names
-Student ID Number (8 or 9 digits)
- Last four digits of Social Security number
- Date of birth
- Your return address
- Your daytime phone number - (optional: e-mail address)
- The name and complete address of each recipient
- Specify what type of information you need verified, i.e., current enrollment status, enrollment dates, or degrees
- Your legal signature to authorize the release of your verification

To place your request in person, please bring your photo identification with your information to 320 Lincoln Tower during normal Hours of Operation.
