If you wish to check the status of a previously mailed transcript request, you may call  (614) 292-0300.

We strongly encourage you to order your transcript on-line, however we will accept requests by mail.

Transcript requests cannot be taken by phone or by fax.

Although it can vary, it generally takes five business days from the date we receive a mailed request to the date we put our response in the mail.

 

To place a request, click on a link below to open up a request form which is available in Adobe format or html. You may then print and submit a completed, signed form.

To place your request by mail, submit the request form with your signature, make sure it is completely filled out, accompanied with your check and mail to:

All requests must contain all information on the form. Missing information could delay or nullify your request.

* We do not accept Credit Card or Cash payments if you are submitting your request by mail. Checks only please.



* Effective June 22, 2009: we will no longer accept cash payments. In-person requests can be paid by check ONLY, You may order your transcript on-line and select the option to pick it up in our office with a credit card.

If you wish to pay by Credit Card, order a Rush Transcript, or use Fed Ex you should use our online service. These options are not available by mail.
