Ordering a Transcript

The Ohio State University (Columbus, Lima, Mansfield, Marion, Newark, and the Agricultural Technical Institute, Wooster, Ohio) is accredited by the North Central Association of Colleges and Secondary Schools as a degree-granting institution at the associate, baccalaureate, masters, professional and doctoral levels.

Note to current students: You have access to your grades via the University Registrar’s web site at www.ureg.ohio-state.edu under On-Line Services.

If you wish to check the status of a previously mailed transcript request, you may call 
(614) 292-8500.The student must sign transcript requests, so they cannot be taken by phone.

Although it can vary, it generally takes five business days from the date we receive a request to the date we put our response in the mail. Faster service, which includes faxing when requested, is available for a fee and is described below.

To place a request, print and complete a  Transcript Request Form or you may write a letter to send by mail or fax, or you may come in to complete and sign a form.

To place your request in person, come to Room 320 Lincoln Tower between 8:00 AM and 5:00 PM Monday through Friday. We are open until 6:00pm on Wednesdays and we are closed most holidays.

Fax: (614) 292-2363 or (614) 292-8700 (See Fax instructions below) Please do not fax to both numbers or fax more than once without adding a note that you are faxing more than once. The reason is that if we receive duplicate requests you may be charged for each one.

All requests must contain the following information. Missing information could delay your request

Include:

  1. Your Full name

  2. Any former names

  3. Social security number

  4. Date of birth

  5. Dates of enrollment

  6. Your return address

  7. Your daytime phone number - (optional: e-mail address)

  8. The name and complete address of each recipient (Your name and address if you want it sent to you.)

  9. Your legal signature to authorize the release of your transcript

  10. $7.00 for each complete transcript ordered 

  11. Special instructions. If transcript is being sent to yourself so you can forward it to other schools be sure to request separate signed sealed envelopes. Most schools require this and we only do this upon request.

Please specify if you need your request to be held for current quarter grades, degrees, grade changes, or if you need any type of special handling such as placing transcripts that are being mailed to the student in separate signed sealed envelopes. If your name has changed since your last enrollment, we will add your new name to our database. Your transcripts will be issued under your current name with your previous name also shown.

Rush processing:

For an additional $10.00 fee, we will process your request within one business day, but most often the same day we receive it, if it is received by 3:30.There is a limit of five transcripts per rush fee. The transcripts will be put in the mail by the end of the day, or you may place your request in person and take them with you. Please indicate on your request that you want rush-processing service, and provide a phone number where you can be reached if there is a problem with processing your rush request.

Express mail service:

We will also use express delivery service if you include payment to cover the additional postage fees.  International prices vary.  Within the Continental United States, next day delivery through DHL Worldwide Express is $8.19, with the last pick-up at 5:00 PM.  Express delivery through the Post Office is the only way to deliver to a P.O. Box address.  The current cost for domestic express mail through the Post Office is $13.65, with the last pick-up in the early afternoon. Any requests for express mail through the post office received after 11:00 AM may not be picked up until the following business day.  All fees are subject to change.  If requested, we will use the appropriate mail service after the request goes through our regular processing, which again, is usually within five business days, but varies. If you do not specify to charge the rush fee we will assume an express delivery also means rush service in our office and we will charge the $10.00 rush fee.

Fax service:

We will fax your documents only if you pay the rush fee and request fax service on your signed request. You must provide both the fax number and the mailing address of the person or organization to which the fax is directed. We must mail the original transcript to the fax recipient or it cannot be faxed.

You may use MasterCard or Visa to pay the fees for your faxed request. These are the only two credit cards we accept. To charge the fees, provide your MasterCard or Visa Account number, the expiration date on the card, and the signature of the cardholder. Please specify in your letter how much we are authorized to charge. If you are not sure of the total, please make an estimate and state a maximum amount you are willing to accept.

Q: What is an “Official” Transcript?

A: All transcripts that we produce in response to a student’s request are “Official” as far as we are concerned. They all are on special paper with the Registrar’s signature, the date and the school seal. Any transcript that we provide to the student will also indicate “ISSUED TO STUDENT”. Some agencies do not consider these to be official. Be sure to check with the agency that wants the transcript to see what they consider to be official. Most schools will accept transcripts that are issued to the student only if they are placed in an envelope that is sealed and has the Registrar’s signature over the seal of the envelope. We will provide them in this manner if you request that your transcripts be issued in Signed and Sealed Envelopes.

Q: Can I get an unofficial transcript for my records?

A: You may request a transcript for your records, but you will have to pay the $7.00 fee and it will actually be an Official Transcript Issued To Student. If you have a current OSU e-mail account, you can print your advising report from our web site at no charge. The advising report contains all of the information on a transcript plus more. Q: What kind of special handling is available?

A: In addition to our Rush and fax services, we will place the transcripts in separate signed, sealed envelopes to mail to the student upon request. If the transcript is being sent to a foreign government that requires it, we will notarize the transcript before we mail it as long as you indicate this in your request. If you have forms that need to be completed and mailed with the transcript, we will do so. However, we cannot send official documents from other sources with our transcripts. You will have to send them separately.

Q: Do I have to order two transcripts to get my OSU undergraduate and OSU graduate records sent?

A: No. Our transcripts are complete records of your entire enrollment at OSU, and would include any transfer credits accepted. However, we cannot issue copies of transcripts you sent to OSU as part of your admission application. We also cannot issue partial transcripts if you want your graduate record only, and also completed undergraduate work here.

Q: Can you guarantee when I will get my transcript?

A: The turnaround time of within five business days is an estimate of how long the request will be in our office before the response is mailed. It is not a guarantee. Frequently it will be less than five business days, but occasionally it will be more. We make every effort to keep our turnaround within the five days. This does not include the time it spends in the mail after it leaves our office.

Q: If I pay the rush fee and the express mail fee with the transcript fee, am I guaranteed to receive my transcript the next day?

A: In most cases, you should get it the next day, but we cannot guarantee it.

Q: Will you let me know if there is a problem in processing my request?

A: If you requested rush service and provided a phone number in your request, we will call you to tell you about any problems. If you provide an e-mail address we may contact you be e-mail as well. If you do not request rush service, we will contact you by regular mail. If you do not give us your current mailing address, we will use the last address we have in our records for you.

Q: What happens if my record has a “hold” on it?

A: If you have an outstanding financial obligation to OSU, such as parking tickets, missing library books, or being behind in student loan payments collected by OSU, your record may have a financial hold. There are other types of holds, as well. If your record has a hold, transcripts will not be issued until the hold is removed. If all other requirements are met, the transcripts will automatically print and be mailed after the hold is cleared, whether you clear it immediately or years later. If you provided your current address, you will be notified if there is a hold on your record when you request transcripts. You may send a written request to cancel your transcript request if you will be unable to clear your hold right away.

Q: Will you issue my transcripts then bill me?

A: No. The payment must accompany the request. If there is no payment with the request, we will send a post card asking for payment. Only after the payment is received, will the transcript be produced.

Q: Can I fax my request to you and then call you with my credit card number to charge the fees?

A: No. We cannot take credit card numbers by phone. We need the signature of the cardholder with the credit card number and the expiration date, so it needs to be included on your faxed request.

Q: Can I fax my request to you and then come in to pick up my transcript in person?

A: No. We are unable to produce transcripts and then hold them. If you want to pick up your transcript, you would need to come in to place your request and pay the rush fee, then wait for the transcript to print. There is no pick-up service without a rush fee.

Q: When I request rush service and pay the $10 rush fee, do you send it via express mail?

A: No. The $10.00 RUSH Service Fee is for shortening the amount of time the request is in our office before the transcript or verification is mailed. It also includes faxing if desired. It does not include express mailing. We mail the response via first class mail the same day or within one business day instead of our regular estimated five days.

Q: Can I get express mail without paying the rush fee, too?

A: Yes. You can pay the postage amount for the type of express mail you choose, and request that we not charge the rush fee then we will use that express service when your transcript leaves our office after going through the regular processing, which can take about 5 business days. 

Q: Please explain how much I have to pay for rush transcripts.

A: You pay the $7 transcript fee for each transcript and the $10 rush fee for each group of five or less transcripts. To rush one transcript, the fee total is $17.00.To rush two transcripts, the fee is $24.00.This is calculated by adding the total transcript fee of $14 plus one rush fee of $10.To rush five transcripts, the fee is $45.00.To rush six transcripts, the fee is $62.00.
($7.00 x 6 transcripts, then add two rush fees.$42+$20=$62).

Q: Can I have my transcript faxed to someone else and mailed to myself?

A: Not one transcript. The faxed transcript must be mailed to the fax recipient. You are welcome to request two transcripts so a second one can also be mailed to you.

Q: Can I have my transcript faxed to myself at work but mailed to my home address?

A: Yes. You are the recipient for both the fax and the hard copy.

Q: What happens if I only provide a fax number in my request, and not the mailing address?

A: We will attempt to contact you to obtain the mailing address. If we are unable to reach you, we will enter the information on our system but will not produce the transcript until we are provided with the complete mailing address.

Q: Do I pay the same for express mail to foreign countries as I would for express mail to a destination within the United States?

A: No. We have to call to find out the current rates and delivery times for international mail. It is not the same as domestic mail, and it varies by the country of destination.
