OSU SIS Reporting Access Request Form

To request access to SIS ODS, eReports or Snapshots, please complete the Access Request Form available at: http://oit.osu.edu/hrfin/securityandworkflow.html

The link to the form called "OSU SIS Reporting Access Request Form" is located in Sequence (Line) 8. Authorized System Access Contact in the Reporting Systems Access column (far right).

The form should be submitted by the Authorized Security Contact or supervisor.

Please be sure to check the User Modules for which access is requested. Once submitted the form will be forwarded to the appropriate SIS Reporting Central Approver.

Please be sure to submit the form using the "Submit" button at the bottom of the form.
